Comprehensive Care Management Programs

Our Comprehensive Care Management Programs (CCMP) are focused on helping people of all ages living with long-term illnesses. We bring them emotional and practical support that helps them maintain their independence, manage their lives, and deal with the challenges they face daily.

Services are provided by CCMP and are made possible by state and federal funds through state waiver programs. There are no out-of-pocket expenses for any of our clients.

We are leaders in the field of case management and palliative care. Our team approach, offering an exhaustive range of lifestyle services and up-to-date medical direction from knowledgeable clinicians, ensures that each of our clients receives the highest quality support to live well and independently, for as long as possible.

HIV/AIDS Case Management Program (HACM)

OUR APPROACH

Quality-of-life coaching – We coordinate just about every aspect of living successfully with HIV/AIDS, with a focus on life improvement. Whatever it takes for our clients to achieve the best outcome, we attend to it.

Advocacy – There are countless resources available to our clients, and we pursue them diligently. They include:

  1. Medical
  2. Legal
  3. Financial
  4. Transportation
  5. Housing/Utilities/Food

Education – HIV/AIDS is greatly misunderstood. We inform our clients about all there is to know, and about everything new that comes along – on such subjects as disease process, transmission and health options.

Partners for Children

Partners for Children is a pediatric palliative care state waiver program, approved in October of 2009, that offers services to children in need through our Comprehensive Care Management Programs at San Diego Hospice (CCMP).

This program provides palliative case management type services to Medi-Cal and CCS (California Child Service) approved children with life-threatening illnesses. It allows them to receive palliative services concurrent with curative treatments. Services to eligible children and their families includes:

  • Care coordination
  • Expressive therapies such as creative art, music, massage and play therapy
  • Family education and training on palliative care issues, care needs, treatments and use of equipment
  • Respite care
  • Support counseling for child and family

Medical conditions that may qualify a child to receive services include:

  • Cancer
  • Cystic Fibrosis
  • Brain or head injuries
  • Spinal muscular atrophy
  • Duchene’s muscular dystrophy dependent on a ventilator
  • Intestinal problems and dependent on IV nutrition
  • Liver or bowel transplant
  • Leukodystrophies
